§ 66-56. Same—Procedure.

Every person required to procure a permit under the provisions of this article or any ordinance or law of this municipality relating to the subject matter of this article shall submit a written application for such permit to the city clerk. The written application shall state the following:

(1)

The name and address of the applicant.

(2)

The purpose for which the application is submitted.

(3)

The nature and location of any work proposed to be done and the name of the person or firm who is to do such work.

(4)

The estimated cost of the proposed work.

(5)

The date of commencement and date of completion of the proposed work, and other date or plans as may reasonably be required by the city clerk or the street superintendent.
